Once you have checked the box below to acknowledge your acceptance of my terms, you will be presented with an intake form that consists of four parts. You are expected to complete each step thoughtfully and honestly. They contain the principles by which your devotion shall be measured...and maintained. Completing this form will require less than 5 minutes of your time and is a necessary first step to demonstrate both intent and devotion. This is the only route through which you may enter the coil.
